If your child has not had these shots please call your primary care physician to make an appointment to take care of this.I will need a copy on file before the start of the 2014-15 school year.Click here to download the packet.
Also, If your child plans to participate in a school sponsored sport in the fall he/she will need a current sports physical exam as well as a signed preparticipation form prior to starting practice.
